Коронарный атеросклероз остается ведущей причиной смертности, в основе патогенеза коронарного атеросклероза лежит воспаление. Интерлейкин-6 (ИЛ-6) − ключевой провоспалительный цитокин. Полиморфизм rs1800795 гена интерлейкина-6 (IL6) может влиять на уровень ИЛ-6, однако данные о его ассоциации с атеросклерозом противоречивы, особенно для российской популяции.</p></sec><sec><title>Цель</title><p>Цель: комплексно оценить ассоциацию полиморфизма rs1800795 гена IL6, уровня ИЛ-6 в плазме с наличием и тяжестью коронарного атеросклероза у пациентов российской популяции.</p></sec><sec><title>Методы</title><p>Методы. В исследование включены 476 пациентов, которым проводилась коронароангиография. Сформированы группы: с атеросклерозом (стеноз ≥50%, n=384) и без него (n=92). Методами ПЦР в реальном времени генотипирован полиморфизм rs1800795, уровень ИЛ-6 определен иммуноферментным анализом. Тяжесть атеросклероза оценена по количеству пораженных артерий и индексу GensiniScore.</p></sec><sec><title>Результаты</title><p>Результаты. Уровень ИЛ-6 был значимо выше в группе с атеросклерозом (3,56 vs 2,08 пг/мл; р=0,009) и ассоциировался с его наличием после коррекции на факторы риска развития атеросклероза и прием статинов (ОШ=4,26; 95% ДИ: 1,22–14,81). Концентрация ИЛ-6 достоверно возрастала с увеличением количества пораженных артерий (р=0,02) и слабо коррелировала с индексом GensiniScore (r=0,15, p=0,04). Носители генотипа С/С имели достоверно более низкие уровни ИЛ-6 (3,76 vs 5,26 пг/мл, р=0,04). Генотип С/С был ассоциирован с меньшей вероятностью атеросклероза при однофакторном анализе (ОШ=0,55; 95% ДИ: 0,32–0,95), но не после многофакторной коррекции (ОШ=0,57; 95% ДИ: 0,31–1,05).</p></sec><sec><title>Выводы</title><p>Выводы. Повышенный уровень ИЛ-6 является независимым фактором риска наличия и тяжести коронарного атеросклероза. Генотип С/С rs1800795 ассоциирован со сниженным уровнем ИЛ-6 и демонстрирует тенденцию к защитному эффекту, однако эта ассоциация не является независимой от традиционных факторов риска развития атеросклероза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Coronary atherosclerosis, driven by inflammation, remains a leading cause of mortality. Interleukin-6 (IL-6) is a key proinflammatory cytokine. The rs1800795 polymorphism of the IL6 gene may influence IL-6 levels; however, data on its association with atherosclerosis are conflicting, particularly for the Russian population.</p></sec><sec><title>Objective</title><p>Objective: to comprehensively assess the association of the gene IL6 rs1800795 polymorphism and plasma IL-6 levels with the presence and severity of coronary atherosclerosis in patients from the Russian population.</p></sec><sec><title>Methods</title><p>Methods. The study included 476 patients undergoing coronary angiography. Groups were formed: with atherosclerosis (stenosis ≥50%, n=384) and without (n=92). The rs1800795 polymorphism was genotyped using real-time PCR, and IL-6 levels were determined by enzyme-linked immunosorbent assay (ELISA). Atherosclerosis severity was assessed by the number of affected arteries and the GensiniScore.</p></sec><sec><title>Results</title><p>Results. IL-6 levels were significantly higher in the atherosclerosis group (3.56 vs. 2.08 pg/mL; p=0.008) and were associated with its presence after adjustment for atherosclerosis risk factors and statin use (OR=4.26; 95% CI: 1.22–14.81). IL-6 concentration significantly increased with a greater number of affected arteries (p=0.02) and showed a weak correlation with the GensiniScore (r=0.15, p=0.04). Carriers of the C/C genotype had significantly lower IL-6 levels (3.76 vs. 5.26 pg/mL, p=0.04). The C/C genotype was associated with a lower likelihood of atherosclerosis in the univariate analysis (OR=0.55; 95% CI: 0.32–0.95) but not after multivariate adjustment (OR=0.57; 95% CI: 0.31–1.05).</p></sec><sec><title>Conclusions</title><p>Conclusions. Elevated IL-6 level is an independent risk factor for the presence and severity of coronary atherosclerosis.
