Около 64 млн человек в мире страдают хронической сердечной недостаточностью, и среди них увеличивется доля лиц с сохранной фракцией выброса. Тщательное изучение генетических полиморфизмов позволит выявлять группы риска, определять фармакогенетические подходы и персонифицировать ведение пациентов. В настоящее время изучено множество генов-кандидатов заболеваний сердечно-сосудистой системы, однако отсутствуют исследования пациентов с сохранной фракцией выброса, что подчеркивает необходимость поиска и тщательного отбора генов-кандидатов в развитии патогенетических изменений, в частности диастолической дисфункции левого желудочка. Одним из генов-кандидатов является ген GNB3, кодирующий β3 субъединицу G-белка (GNB3).</p></sec><sec><title>Цель</title><p>Цель: оценить частоту различных полиморфных вариантов гена GNB3 и их роль в развитии диастолической дисфункции.</p></sec><sec><title>Методы</title><p>Методы. Проведено одномоментное исследование, в которое были включены 87 женщин в поздней постменопаузе. Наличие диастолической дисфункции левого желудочка диагностировалось при трансторакальной эхокардиографии, оценены уровни адреномедуллина, ренина, N-концевого предшественника «мозгового» натрийуретического пептида, общего холестерина, липопротеинов низкой плотности, глюкозы в сыворотке крови. Полиморфизм гена GNB3 825 C&gt;T (rs5443) оценивали методом полимеразной цепной реакции.</p></sec><sec><title>Результаты</title><p>Результаты. Наличие аллеля C ассоциировалось с более высоким риском атеросклероза, что объясняется тем, что носители генотипов СС и СТ имели более высокий уровень ЛПНП (p=0,037) и чаще страдали от ожирения (p=0,037), чем носительницы генотипа ТТ. Анализ ремоделирования не показал статистически значимых различий среди трех возможных генотипов, но наличие аллеля С предрасполагало к дилатационным изменениям, а присутствие аллеля Т повышало вероятность выявления гипертрофии миокарда левого желудочка. Было показано отсутствие статистических различий в группах с диастолической дисфункцией левого желудочка и без нее, а также с различными уровнями адреномедуллина, ренина, N-концевого предшественник «мозгового» натрийуретического пептида.</p></sec><sec><title>Выводы</title><p>Выводы. Выявленные взаимосвязи полиморфизма GNB3 и склонности к развитию сердечно-сосудистых заболеваний могут способствовать развитию персонифицированного подхода к диагностике и лечению пациентов с сердечной недостаточностью. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Globally, about 64 million people have been reported to suffer from heart failure, and there with increasing the proportion of heart failure with preserved ejection fraction. GNB3 gene encoding β3 subunit of G-protein may be one of the candidate genes for pharmacogenetic approaches to heart failure management.</p></sec><sec><title>Aim</title><p>Aim: to evaluate the frequency of various polymorphisms of the guanine nucleotide-binding protein subunit 3 gene and their role in the development of diastolic dysfunction.</p></sec><sec><title>Methods</title><p>Methods. A cross-sectional study enrolled 87 late postmenopausal women. All patients underwent transthoracic echocardiography, serum the levels of adrenomedullin, renin, NT-proBNP, total cholesterol, low-density lipoprotein, and glucose were assessed. Polymorphisms of the guanine nucleotide-binding protein subunit 3 gene 825 C &gt; T (rs5443) were evaluated by polymerase chain reaction.</p></sec><sec><title>Results</title><p>Results. The analysis showed that the C allele presence increased the risks of atherosclerotic changes by predisposing to elevated LDL levels and obesity. Although remodeling analysis showed no statistically significant differences between the three possible genotypes, the C allele predisposes more to dilated changes, and the T allele might increase the likelihood of left ventricular myocardial hypertrophy. There were no statistical differences between groups with and without left ventricular diastolic dysfunction and between patients with different levels of adrenomedullin, renin, NT-proBNP.</p></sec><sec><title>Conclusions</title><p>Conclusions. The revealed interrelationships of polymorphisms and propensity to cardiovascular conditions can contribute to the development of personalized approach to the diagnosis and treatment of patients with heart failure.
