   Семейная средиземноморская лихорадка (ССЛ) – аутовоспалительное заболевание, развивающееся вследствие гиперактивации инфламмасомы, вызванной патогенными вариантами в гене MEFV, которая приводит к гиперпродукции ИЛ-1b и ИЛ-18. ССЛ характеризуется высокой вариабельностью экспрессивности и пенетрантности. В исследование вошли 1336 образцов, 1271 из них имеет варианты в гене MEFV. 160 образцов были разделены на 3 группы: 95 пациентов с вариантами гена MEFV (ССЛ), 49 пациентов с системной красной волчанкой (СКВ) и 16 условно здоровых доноров (УЗД). Во всех группах проведено исследование уровня СРБ, ИЛ-6, ИЛ-8 и ИЛ-18 методом ИФА. По уровню ИЛ-8 не было получено статистически значимых различий, уровень остальных цитокинов в группе ССЛ выше, чем в группе УЗД (р < 0,0001), а уровни СРБ и ИЛ-18 выше, чем в группе СКВ (р < 0,0001). Уровень ИЛ-18 во всех подгруппах ССЛ выше, чем в группе УЗД (р < 0,0001). Уровень ИЛ-18 у пациентов с агрессивными вариантами выше, чем у носителей других вариантов (р = 0,0411). Уровень провоспалительных маркеров пациентов с ССЛ определяется генотипом и вариантами гена MEFV.

   Familial Mediterranean fever (FMF) is an autoinflammatory disease manifested by recurrent fever with signs of systemic inflammation caused by MEFV gene pathogenic variants that lead to inflammasome hyperactivation and hyperproduction of IL-1b and IL-18. FMF is a disorder with variable expressivity and penetrance. The study included 1336 samples, 1271 samples were positive for pathogenic variants in MEFV gene. 160 samples divided into 3 groups: 95 patients (FMF) with different MEFV genotypes (5 subgroups), 49 patients with systemic lupus erythematosus (SLE) and 16 samples as a group of healthy donors (HD). Concentrations of CRP, IL-6, IL-8 and IL-18 were measured for all samples. Concentrations of CRP, IL-6 and IL-18 in FMF group was significantly higher than in HD and SLE groups (р < 0.0001). Concentration of IL-18 was significantly higher in all FMF genotypes subgroups than in HD. Comparing IL-18 levels between different pathogenic MEFV variants samples with «aggressive» variants (p.Met694Val and p.Met680Ile) characterized by increased concentration of IL-18 (р = 0.0411).
