Данное заболевание включено в перечень заболеваний для расширенного неонатального скрининга в Российской Федерации (РФ) с января 2023 года. В результате первичного скрининга в региональных центрах была сформирована группа новорожденных для этапа подтверждающей диагностики в ФГБНУ Медико-генетический научный центр имени академика Н.П. Бочкова (ФГБНУ МГНЦ) (N=27). При подтверждающей диагностике проводилось повторное определение концентрации аминокислот и ацилкарнитинов крови методом тандемной масс-спектрометрии (ТМС) (ретест), а также определение концентрации органических кислот мочи методом газовой хроматографии-масс-спектрометрии (ГХ-МС) и молекулярно-генетическое исследование кодирующей области генов ETFA, ETFB, ETFDH. Диагноз ГА2 был подтвержден у 6 пациентов. Частота ГА2 в РФ составила 1:205233 живых новорожденных, что сопоставимо с данными по Европе и США. При подтверждающей биохимической диагностике выявлено, что наиболее информативным биомаркером для постановки диагноза является повышение концентрации среднецепочечных ацилкарнитинов (С8-С12) и определение ранее неиспользованного в алгоритме скрининга ГА2-индекса (соотношение [C4xC5x C8xC14]/[C0xC3]), повышение которого было выявлено у 5 из 6 пациентов. Молекулярные причины развития ГА2 выявлены у всех 6 пациентов. У пяти новорожденных обнаружены биаллельные мутации в гене ETFDH (четыре в FAD-связывающем домене), у одного новорожденного выявлены ранее не описанные нуклеотидные варианты в гене ETFA. На момент обследования симптомы присутствовали у трех новорожденных (все носители мутаций в гене ETFDH). У двух пациентов с тяжелой формой ГА2 (один с летальным исходом) был выявлен вариант c.652G&gt;A (p.Asp218Asn) в гомозиготном состоянии, что позволяет предположить его ассоциацию с тяжелым неонатальным фенотипом ГА2. По результатам скрининга можно сделать вывод о том, что мутации в гене ETFDH могут быть ассоциированы с более тяжелым фенотипом ГА2, чем предполагалось ранее, а также рекомендовать расчет ГА2-индекса [C4xC5 x C8xC14]/[C0xC3] для повышения эффективности биохимического скрининга на ГА2.</p></abstract><trans-abstract xml:lang="en"><p>Glutaric aciduria type 2 (GA2), or multiple acyl-CoA dehydrogenase deficiency (MADD, OMIM 231680), is a rare life-threatening disorder associated with impaired metabolism of fatty acids and amino acids. GA2 has been included in the expanded neonatal screening program in the Russian Federation since January, 2023. Following the primary screening in the regional centers, a group of newborns underwent the second stage of confirmatory diagnostics at the Research Center for Medical Genetics (N = 27). Concentrations of amino acids and acylcarnitines in the blood were remeasured using MS/MS, organic acids in the urine were analyzed using GC-MS, and a molecular genetic study of the coding regions of the ETFA, ETFB, and ETFDH genes was conducted. GA2 was confirmed in 6 patients. The frequency of GA2 was 1 in 205,233 live births, which was comparable to the data from Europe and the USA. An increase in the concentration of medium-chain acylcarnitines (C8-C12) and determination of «GA2 index» ([C4xC5 x C8xC14]/[C0xC3], which has not been previously used in the screening algorithm) were found to be informative biomarkers for GA2. The «GA2 index» was increased in 5 out of the 6 patients. Five newborns had biallelic mutations in the ETFDH gene (four in the FAD-binding domain), and one newborn had previously undescribed variants in the ETFA gene. At the time of examination, symptoms were present in three newborns (all carriers of the ETFDH mutations). Two patients with a severe form of GA2 (one with a lethal outcome) were homozygous carriers of the c.652G&gt;A (p.Asp218Asn) variant, suggesting an association between this variant and a severe neonatal GA2 phenotype. Thus, mutations in the ETFDH gene may be associated with a more severe phenotype of GA2 than previously estimated.
